Please make sure you complete all pages and sign the form agreeing to the terms of the agreement.
If you will be required to receive a Form1099, you will be sent a request to onboard on our 1099 portal. Here you will be able to register to see your payments and set up direct deposit payments.
If you are Incorporated and do not require a Form 1099, we have the option to issue your payments via ACH transfer. If you would like to be paid via ACH payment, please complete the ACH Authorization Form on the link below.
